Business continuity planning is a mandatory control across SOC 2, ISO 27001, and HIPAA. Despite this, it is one of the controls most frequently found deficient during audits. The pattern is predictable: an organization creates a business continuity plan document during initial certification, files it away, and never tests it. When the auditor asks for test results, tabletop exercise records, or evidence that the plan was reviewed in the past twelve months, the organization has nothing to show.

A business continuity plan that passes audits is not a static document. It is a living program with documented analysis, defined recovery objectives, tested procedures, and evidence of regular review. This guide covers how to build one correctly.

BCP vs. DRP: Understanding the Difference

Auditors distinguish between Business Continuity Plans (BCP) and Disaster Recovery Plans (DRP), and your documentation should reflect this distinction.

A Business Continuity Plan addresses how the organization will maintain essential business functions during and after a disruption. It covers people, processes, communications, facilities, and technology. The scope extends beyond IT systems to include manual workarounds, alternate work locations, vendor dependencies, and communication chains.

A Disaster Recovery Plan is a subset of the BCP focused specifically on restoring IT systems and data after a disruption. It covers system recovery procedures, backup restoration, failover mechanisms, and the technical steps to bring infrastructure back to operational state.

Most compliance frameworks require both. SOC 2 evaluates business continuity under the Availability criteria. ISO 27001 addresses it through Annex A controls A.5.29 (Information Security During Disruption) and A.5.30 (ICT Readiness for Business Continuity). HIPAA requires a contingency plan under the Security Rule (164.308(a)(7)), which must include a data backup plan, disaster recovery plan, and emergency mode operation plan.

Business Impact Analysis (BIA)

The Business Impact Analysis is the foundation of your entire business continuity program. Without a BIA, your recovery priorities are arbitrary and your RTO/RPO targets are guesses. Auditors across all three frameworks expect a documented BIA.

A BIA identifies and prioritizes business functions based on the impact of their disruption. For each critical function, the BIA documents:

Function description and owner: What the function does and who is responsible for it.

Dependencies: Systems, data, personnel, vendors, and facilities required for the function to operate.

Impact of disruption: Financial impact (revenue loss, penalty exposure, cost of manual workarounds), operational impact (downstream process failures), regulatory impact (compliance violations, reporting failures), and reputational impact (customer trust, market position).

Maximum Tolerable Downtime (MTD): The longest period the organization can sustain the loss of this function before the impact becomes unacceptable.

Recovery priorities: Based on impact and MTD, a ranked list of functions that determines the order of recovery.

The BIA directly informs your Recovery Time Objectives and Recovery Point Objectives for each critical system supporting these business functions.

RTO and RPO: Defining Recovery Objectives

Recovery Time Objective (RTO) is the maximum acceptable time to restore a system or function after a disruption. An RTO of four hours means the system must be operational within four hours of a disruption event.

Recovery Point Objective (RPO) is the maximum acceptable amount of data loss measured in time. An RPO of one hour means you can tolerate losing up to one hour of data, which dictates your backup frequency (in this case, backups must occur at least hourly).

RTO and RPO must be defined for each critical system, not as blanket organizational targets. A customer-facing SaaS application might require an RTO of one hour and RPO of 15 minutes, while an internal reporting system might tolerate an RTO of 24 hours and RPO of 24 hours.

Auditors verify that:

  • RTO and RPO targets are documented for all critical systems
  • Backup frequency aligns with stated RPO targets (if your RPO is one hour but you back up daily, you have a gap)
  • Recovery testing demonstrates that RTO targets are achievable (if your RTO is four hours but your last DR test took twelve hours, you have a gap)
  • RTO and RPO targets are derived from the BIA, not assigned arbitrarily

BCP Documentation Structure

A business continuity plan that satisfies auditors across SOC 2, ISO 27001, and HIPAA should include the following sections:

1. Purpose, Scope, and Objectives. Define what the plan covers, which business functions and systems are in scope, and the overall recovery objectives.

2. Roles and Responsibilities. Define the BCP team, including an executive sponsor, BCP coordinator, technical recovery leads, and communications lead. Include contact information and alternates for each role. HIPAA specifically requires identification of responsible parties.

3. Business Impact Analysis Summary. Reference or include the full BIA, showing critical business functions ranked by priority with MTD, RTO, and RPO for each.

4. Risk Assessment. Document the threats and scenarios the plan addresses: natural disasters, infrastructure failures, cyberattacks (including ransomware), vendor outages, pandemic/workforce unavailability, and facility loss. Map each scenario to affected business functions.

5. Recovery Strategies. For each critical function and its supporting systems, document the recovery approach: automated failover, backup restoration, manual workaround, alternate site operations, or vendor-provided recovery.

6. Recovery Procedures. Step-by-step technical and operational procedures for executing recovery. These should be detailed enough that someone unfamiliar with the plan could follow them. Include system recovery sequences (which systems must be restored first based on dependencies), backup restoration procedures, failover activation steps, and data validation procedures post-recovery.

7. Communication Plan. Define how the organization communicates during a disruption: internal notifications (employees, management, board), external notifications (customers, partners, regulators), media communications, and escalation procedures. HIPAA-covered organizations must include breach notification procedures when a disruption involves potential PHI exposure.

8. Vendor Dependencies. List critical third-party services, their SLAs, and contingency plans for vendor failures. Include vendor contact information and escalation paths.

9. Testing and Exercise Schedule. Document the testing cadence, types of tests, and review schedule (covered in detail below).

10. Plan Maintenance. Define the review and update cycle, triggers for off-cycle updates (organizational changes, new systems, lessons from incidents or tests), and version control procedures.

Testing Requirements

Testing is where most organizations fail their audits. A plan that has never been tested provides no assurance. Auditors across all three frameworks require evidence that the BCP has been tested.

Types of Tests

Tabletop exercises: The most common and most expected test type. A facilitated discussion where the BCP team walks through a specific disruption scenario, discussing decisions, actions, and gaps without actually executing recovery procedures. Tabletop exercises should be conducted at least annually. They are low-cost, engage leadership, and reliably surface gaps in the plan.

Walkthrough tests: Team members walk through recovery procedures step by step, verifying that documentation is accurate and complete without actually failing over systems. Useful for validating that procedures are current and actionable.

Functional tests: Actual recovery of specific systems in a test environment. Restore from backup, activate failover, and verify data integrity. These tests validate that RTO and RPO targets are achievable and that backups are viable.

Full-scale tests: Complete activation of the business continuity plan, including failover of production systems to disaster recovery environments. These are resource-intensive and carry operational risk, so they are less frequent but provide the highest assurance.

Testing Cadence

At minimum, auditors expect:

  • Annual tabletop exercise involving the BCP team and key stakeholders
  • Annual backup restoration test verifying that backups can be restored successfully and data is intact
  • Quarterly backup verification confirming that automated backups are completing successfully
  • Test after significant changes to infrastructure, applications, or organizational structure

Documenting Test Results

Every test must produce documentation that includes:

  • Date, participants, and scope of the test
  • Scenario tested (for tabletop exercises)
  • Results, including systems recovered, recovery time achieved, and data integrity validation
  • Gaps or issues identified during the test
  • Action items with owners and deadlines for addressing identified gaps
  • Sign-off by BCP coordinator or executive sponsor

This documentation is primary audit evidence. Without it, the test did not happen from an auditor's perspective.

Framework-Specific Requirements

SOC 2

The Availability criteria (A1.2) requires that organizations design, develop, and implement recovery procedures to meet defined recovery objectives. A1.3 requires testing of recovery procedures. Auditors evaluate whether recovery objectives are defined, procedures are documented, tests are conducted, and results demonstrate that objectives are achievable.

ISO 27001

Annex A control A.5.30 (ICT Readiness for Business Continuity) requires that ICT readiness is planned, implemented, maintained, and tested based on business continuity objectives and ICT continuity requirements. ISO 27001 auditors typically expect a more structured approach, with the BCP integrated into the overall ISMS and risk treatment plan. Business continuity risks should appear in your risk register.

HIPAA

The HIPAA Security Rule contingency plan standard (164.308(a)(7)) has five implementation specifications:

  • Data backup plan (Required): Procedures to create and maintain retrievable exact copies of ePHI
  • Disaster recovery plan (Required): Procedures to restore any loss of data
  • Emergency mode operation plan (Required): Procedures to enable continuation of critical business processes while operating in emergency mode
  • Testing and revision procedures (Addressable): Procedures for periodic testing and revision of contingency plans
  • Applications and data criticality analysis (Addressable): Assess the relative criticality of specific applications and data in support of contingency planning

While testing is listed as "addressable" under HIPAA, this does not mean optional. Organizations must implement it or document why an equivalent alternative measure is in place. In practice, every auditor expects testing evidence.

Common Audit Failures

No BIA documented. Without a BIA, auditors question the basis for your recovery priorities and objectives. This is a fundamental gap.

RTO/RPO not defined per system. Blanket recovery objectives without system-level specificity suggest the plan is not actionable.

No test evidence. The single most common BCP audit finding. Organizations that cannot produce dated, documented test results with identified gaps and action items will receive a finding.

Stale plan. A BCP last reviewed or updated more than twelve months ago, or one that does not reflect current systems and organizational structure, demonstrates that the program is not maintained.

Backup restoration never tested. Organizations that back up data but never test restoration are assuming their backups work. Auditors consider untested backups unreliable.
