And yet, most companies approach compliance as a point-in-time exercise. They prepare for the audit, pass the audit, and then let the program coast until the next audit cycle begins. For six or nine or eleven months, nobody is watching the controls. Nobody is verifying that evidence is being collected. Nobody notices when an access review stops happening, when a policy expires without renewal, when a new cloud service is provisioned outside the compliance scope, or when a vendor's SOC 2 report lapses.
Then the auditor arrives and asks for twelve months of evidence. The scramble begins.
This is the problem that compliance monitoring solves -- not as a product category or a vendor pitch, but as an operational discipline. Compliance monitoring is the practice of continuously verifying that your security controls are functioning as designed, that evidence of their operation is being captured, and that deviations are detected and remediated before they become audit findings.

What Is Compliance Monitoring?
Compliance monitoring is the ongoing, systematic process of observing, measuring, and evaluating an organization's adherence to its regulatory, contractual, and policy-based obligations. It encompasses the continuous verification that security controls are operating effectively, that required evidence is being generated and retained, and that deviations from expected behavior are detected, escalated, and resolved.
The distinction between compliance monitoring and periodic auditing is fundamental and frequently misunderstood.
Periodic auditing is a retrospective examination. An auditor arrives at a defined point in time, samples a subset of controls, reviews historical evidence, interviews personnel, and renders an opinion on whether the organization was compliant during the observation period. The audit itself does not prevent non-compliance. It discovers it after the fact.
Compliance monitoring is a prospective and ongoing operation. It runs continuously -- daily, hourly, or in real time depending on the control -- and its purpose is to detect non-compliance as it happens, not months later during an audit. A compliance monitoring program does not replace audits. It ensures that when the auditor arrives, there are no surprises.
Think of it this way: an audit is an X-ray. It reveals what is broken at a point in time. Compliance monitoring is a heart monitor. It watches for problems continuously and alerts you the moment something goes wrong.
What compliance monitoring covers
A comprehensive compliance monitoring program tracks several dimensions simultaneously:
- Control effectiveness -- Are the security controls you have implemented actually working? Is your endpoint detection tool running on all endpoints? Are your firewall rules correctly configured? Is multi-factor authentication enforced on all accounts?
- Evidence generation -- Is the evidence that auditors will eventually request being captured automatically and stored in an accessible format? Are access review records being generated quarterly? Are change management tickets being closed with proper documentation?
- Policy adherence -- Are employees following the policies you have documented? Have all employees completed security awareness training? Have all personnel acknowledged the acceptable use policy? Are access requests following the approval workflow?
- Regulatory requirements -- Are you meeting the specific, prescriptive requirements of the frameworks you are certified against? Are vulnerability scans running at the cadence required by PCI DSS? Are risk assessments being conducted annually as required by ISO 27001?
- Vendor and third-party compliance -- Are your critical vendors maintaining their own compliance certifications? Have any vendor SOC 2 reports expired? Have you received updated Business Associate Agreements from healthcare-adjacent vendors?
Compliance monitoring is not a single tool or a single dashboard. It is an operational capability that integrates across your security tooling, IT infrastructure, HR processes, and vendor management program to provide a continuous, holistic view of your compliance posture.
Why Annual Audits Are No Longer Enough
The annual audit model was designed for a different era of technology and risk. When organizations operated static, on-premises infrastructure with infrequent changes, a yearly examination was a reasonable cadence. The environment changed slowly. Controls, once implemented, tended to stay in place. Evidence was generated through manual processes that, while tedious, were predictable.
That world no longer exists. Modern SaaS companies deploy code to production dozens or hundreds of times per day. Infrastructure is provisioned and decommissioned through code. Employees join and leave at a pace that makes annual access reviews meaningless. Vendors are added to the technology stack with a credit card and a terms-of-service checkbox. The attack surface changes daily.
In this environment, an annual audit is like checking the locks on your doors once a year. The doors may have been wide open for eleven months.
The compliance drift problem
Between audits, organizations experience what practitioners call compliance drift -- the gradual divergence between the documented compliance posture and the actual state of the environment. Compliance drift happens through five primary mechanisms:
Personnel changes. An employee who managed quarterly access reviews leaves the company. Nobody inherits the responsibility. Three quarters pass without a review. The auditor asks for twelve months of access review evidence. You have one quarter.
Infrastructure changes. The engineering team migrates from a single-region AWS deployment to a multi-region architecture. New VPCs, new IAM roles, new data flows. None of these changes are reflected in the compliance scope documentation. Controls that covered the old architecture do not cover the new one.
Tool changes. Your team switches from one monitoring tool to another. The automated evidence collection pipeline that pulled configuration screenshots from the old tool now produces nothing. Nobody notices because nobody is monitoring the monitoring.
Policy decay. Policies are written during audit preparation and then never revisited. Twelve months later, the acceptable use policy references a VPN that was decommissioned, an HR system that was replaced, and an incident response Slack channel that no longer exists.
Vendor drift. A critical vendor's SOC 2 report expires. A subprocessor changes their data processing location. A vendor you classified as low-risk is acquired by a company with a different security posture. None of these changes are detected until the next vendor review cycle -- if one exists at all.
Each of these mechanisms produces gaps that auditors find. And each is preventable with continuous compliance monitoring.
The business reality
Beyond audit risk, the shift toward continuous monitoring is being driven by business requirements. Enterprise customers increasingly require evidence of continuous compliance, not just annual certification. Cyber insurance underwriters are requesting real-time security posture data. Regulatory bodies -- particularly in healthcare and financial services -- are moving toward continuous oversight models. And the cost of audit remediation when months of drift have accumulated is dramatically higher than the cost of catching and fixing issues in real time.
Organizations that monitor continuously report spending 60-70% less time on audit preparation compared to those that rely on point-in-time reviews. The math is straightforward: if your compliance program generates evidence and catches deviations every day, audit prep is a matter of packaging existing data. If it generates evidence only during audit season, audit prep is a reconstruction project.
Compliance Monitoring Requirements by Framework
Every major compliance framework includes explicit requirements for ongoing monitoring. These are not suggestions. They are auditable controls that require evidence of continuous operation.
SOC 2: CC4.1 and CC4.2 (Monitoring Activities)
The SOC 2 Trust Services Criteria include two Common Criteria specifically dedicated to monitoring:
CC4.1 -- COSO Principle 16: "The entity selects, develops, and performs ongoing and/or separate evaluations to ascertain whether the components of internal control are present and functioning."
This criterion requires that your organization has defined how it monitors the effectiveness of its internal controls. The auditor expects to see:
- A documented monitoring strategy that identifies which controls are monitored, at what frequency, and by whom
- Evidence that monitoring activities are actually being performed (not just planned)
- Records showing that monitoring results are evaluated and acted upon
- Defined thresholds or criteria for what constitutes a control failure
CC4.2 -- COSO Principle 17: "The entity evaluates and communicates internal control deficiencies in a timely manner to those parties responsible for taking corrective action, including senior management and the board of directors, as appropriate."
This criterion addresses what happens when monitoring detects a problem. The auditor expects to see:
- A defined escalation path for control deficiencies
- Evidence that identified deficiencies were communicated to appropriate personnel
- Remediation timelines and evidence that deficiencies were corrected
- A mechanism for tracking deficiencies from detection to resolution
Together, CC4.1 and CC4.2 create a monitoring-and-response cycle that must operate throughout the audit period. ISO 27001: Clause 9 (Performance Evaluation)
ISO 27001:2022 dedicates its entire Clause 9 to performance evaluation, which encompasses monitoring, measurement, analysis, and evaluation of the Information Security Management System.
Clause 9.1 -- Monitoring, Measurement, Analysis, and Evaluation requires the organization to determine:
- What needs to be monitored and measured (including information security processes and controls)
- The methods for monitoring, measurement, analysis, and evaluation to ensure valid results
- When the monitoring and measuring shall be performed
- Who shall monitor and measure
- When the results shall be analyzed and evaluated
- Who shall analyze and evaluate these results
ISO 27001 auditors expect a documented monitoring plan that maps each ISMS control to a specific monitoring method, frequency, and responsible party. They will examine whether that plan was followed and whether the results were reviewed by management. HIPAA: Section 164.308(a)(8) -- Evaluation
The HIPAA Security Rule at 45 CFR 164.308(a)(8) requires covered entities and business associates to "perform a periodic technical and nontechnical evaluation, based initially upon the standards implemented under this rule and, subsequently, in response to environmental or operational changes affecting the security of electronic protected health information."
While HIPAA does not prescribe a specific cadence (the regulation says "periodic"), the Office for Civil Rights (OCR) has made clear through enforcement actions and guidance that annual evaluations are the minimum acceptable frequency, and that significant environmental changes -- such as infrastructure migrations, new application deployments, or changes to data flows involving ePHI -- should trigger additional evaluations.
In practice, HIPAA compliance monitoring for SaaS companies handling protected health information should include:
- Continuous monitoring of access controls on systems containing ePHI
- Regular review of audit logs for unauthorized access attempts
- Ongoing verification that encryption is applied to ePHI at rest and in transit
- Monitoring of Business Associate compliance, including BAA currency and vendor security posture
- Tracking of workforce training completion and security awareness

Cross-framework implications
If your organization is certified against multiple frameworks -- which is increasingly common for SaaS companies selling to enterprise and healthcare customers -- the monitoring requirements compound. The good news is that a single, well-designed compliance monitoring program can satisfy the monitoring requirements across SOC 2, ISO 27001, HIPAA, and other frameworks simultaneously. The key is to build your monitoring program around controls rather than frameworks, and then map each monitored control to every framework requirement it satisfies. Building a Compliance Monitoring Program: 7 Steps
Building a compliance monitoring program is not a technology project. It is an operational design challenge. The tools matter, but the process, ownership, and integration with existing workflows matter more. Here is a seven-step approach that works for organizations ranging from 20-person startups to 500-person scale-ups.
Step 1: Inventory your controls and evidence requirements
Before you can monitor anything, you need a complete, current inventory of every control your organization operates and every piece of evidence that your auditors expect to see. This is not the same as your policy library. Policies describe what you intend to do. Controls are what you actually do. Evidence proves you did it.
For each control, document:
- The control description (what it does and what risk it mitigates)
- The frameworks it maps to (SOC 2 CC reference, ISO 27001 Annex A control, HIPAA section)
- The evidence type required (screenshot, log export, configuration file, ticket record, signed acknowledgment)
- The evidence frequency (continuous, daily, weekly, monthly, quarterly, annual)
- The current evidence collection method (automated, semi-automated, manual)
- The control owner (the person responsible for the control's operation)
If you do not have this inventory, start by working backward from your last audit report or readiness assessment. The evidence request list from your auditor is the most practical starting point for identifying what needs to be monitored.
Step 2: Classify controls by monitoring type
Not all controls can or should be monitored the same way. Classify each control into one of four monitoring categories:
Continuous automated monitoring. Controls where the state can be checked programmatically in real time or near-real time. Examples: MFA enforcement status, endpoint protection agent deployment, encryption configuration, firewall rules, IAM policies. These are your highest-priority automation targets.
Periodic automated monitoring. Controls that require automated checks at defined intervals. Examples: vulnerability scan completion, backup verification, access review completion, certificate expiration, vendor SOC 2 report currency. These run on schedules -- daily, weekly, monthly, or quarterly.
Event-triggered monitoring. Controls that should be verified when a specific event occurs. Examples: new employee onboarding (was access provisioned according to policy?), employee offboarding (was access revoked within the required timeframe?), infrastructure change (was the change logged through the change management process?), security incident (was the incident response plan followed?).
Manual periodic review. Controls that cannot be meaningfully automated and require human judgment. Examples: policy content review, risk assessment updates, management review meetings, business continuity plan tabletop exercises. These should still be tracked and scheduled through your monitoring program, even if the review itself is manual.
Step 3: Define monitoring thresholds and alert criteria
For each monitored control, define what "healthy" looks like and what constitutes a deviation. This is where most compliance monitoring programs fail -- not because they lack tools, but because they lack clear definitions of success and failure.
Good monitoring thresholds are specific and measurable:
- MFA enforcement: 100% of user accounts must have MFA enabled. Any account without MFA is a deviation.
- Endpoint protection: 100% of corporate endpoints must have the EDR agent installed and reporting. Any endpoint not reporting for more than 24 hours is a deviation.
- Access reviews: Quarterly access reviews must be completed within 15 business days of the quarter close. A review not completed by day 16 is a deviation.
- Vulnerability remediation: Critical vulnerabilities must be remediated within 7 days of discovery. Any critical vulnerability older than 7 days is a deviation.
- Policy acknowledgment: All employees must acknowledge the information security policy within 30 days of hire and annually thereafter. Any unacknowledged policy past the deadline is a deviation.
These thresholds become the basis for your alerting rules, your dashboards, and your audit evidence. They also become the basis for auditor conversations -- when you can show an auditor a dashboard with defined thresholds and evidence that deviations were detected and remediated, you are demonstrating a mature compliance monitoring program.
Step 4: Assign ownership
Every monitored control needs a named owner -- not a team, not a department, a specific person. Control ownership means responsibility for three things:
- Ensuring the control operates as designed
- Responding to monitoring alerts when the control deviates from its expected state
- Providing evidence of the control's operation to auditors
In practice, control ownership typically maps along functional lines: engineering owns technical controls (infrastructure configuration, code deployment processes, vulnerability management), IT owns operational controls (endpoint management, access provisioning, identity management), HR owns people controls (background checks, security awareness training, onboarding/offboarding procedures), and legal or compliance owns governance controls (policy management, vendor agreements, regulatory tracking).
Document ownership explicitly. When the auditor asks "who is responsible for this control?" the answer should never be "the security team" or "engineering." It should be a specific person's name.
Step 5: Build the evidence pipeline
Evidence collection is the operational core of compliance monitoring. Every monitored control must produce evidence that can be retrieved, reviewed, and presented to auditors. The goal is to build an evidence pipeline that captures this evidence automatically, stores it in a centralized and organized location, and retains it for the duration required by your compliance frameworks.
The evidence pipeline has three components:
Collection. How evidence is generated and captured. For automated controls, this means API integrations that pull configuration states, log exports, scan results, and ticket records from your production systems. For manual controls, this means structured templates and workflows that produce consistent, auditable documentation.
Storage. Where evidence is stored and how it is organized. Evidence should be stored in a centralized system -- not scattered across Google Drive folders, Slack messages, and email threads. It should be organized by control, tagged by framework, and timestamped. Auditors need to be able to trace any piece of evidence back to a specific control at a specific point in time.
Retention. How long evidence is kept. SOC 2 requires evidence for the observation period (typically 3-12 months). ISO 27001 requires evidence for the full certification cycle (3 years). HIPAA requires documentation retention for 6 years. Your evidence pipeline must retain records for the longest applicable period.
Step 6: Establish the monitoring cadence
With controls inventoried, classified, thresholds defined, owners assigned, and the evidence pipeline built, you need to establish the operational cadence of your monitoring program.
A practical monitoring cadence for most SaaS companies:

This cadence is not rigid -- adjust it to your organization's size, risk profile, and framework requirements. The point is that monitoring is an always-on operational function, not a pre-audit activity.
Step 7: Integrate with existing workflows
A compliance monitoring program that exists in isolation will fail. It must integrate with the tools and workflows your teams already use. This means:
- Ticketing integration. When monitoring detects a deviation, it should create a ticket in Jira, Linear, Asana, or whatever your teams use. Not an email. Not a Slack message. A tracked, assignable, measurable work item.
- Communication integration. Alerts and escalations should flow through the channels your team actually watches. If your engineering team lives in Slack, compliance alerts go to Slack. If leadership reviews dashboards in a weekly meeting, compliance metrics go on that dashboard.
- HR integration. Onboarding and offboarding events should trigger compliance workflows automatically. When HR marks an employee as terminated, your monitoring system should verify that access was revoked across all systems within the required timeframe.
- Change management integration. Your monitoring system should be aware of changes happening in your environment. The 5 Pillars of Continuous Compliance Monitoring
A mature compliance monitoring program rests on five pillars. Each addresses a distinct dimension of compliance risk, and each requires different monitoring approaches, tools, and ownership.
Pillar 1: Control effectiveness monitoring
Control effectiveness monitoring answers the question: are your security controls actually working right now?
This is the most technically intensive pillar and the most amenable to automation. It involves continuously checking the configuration and operational state of your security controls against their expected baselines. Examples include:
- Identity and access management. Verifying that MFA is enforced, that password policies meet requirements, that privileged access is restricted to authorized personnel, that service accounts have appropriate permissions, and that no dormant accounts exist.
- Network security. Verifying that firewall rules match the approved configuration, that network segmentation is intact, that ingress and egress rules are correct, and that no unauthorized ports are open.
- Encryption. Verifying that data at rest is encrypted in all storage systems, that data in transit uses TLS 1.2 or higher, that encryption keys are managed according to policy, and that certificates are not expired or expiring.
- Endpoint security. Verifying that endpoint detection and response agents are installed on all endpoints, that they are reporting to the central console, that definitions are up to date, and that no endpoints are in a degraded state.
- Logging and monitoring. Verifying that audit logging is enabled on all in-scope systems, that logs are being forwarded to the central SIEM or log aggregator, that log retention meets framework requirements, and that alerting rules are active.
When control effectiveness monitoring detects a deviation -- an endpoint without EDR, an S3 bucket without encryption, a user account without MFA -- it should generate an alert, create a remediation ticket, and track the deviation until it is resolved.
Pillar 2: Evidence collection monitoring
Evidence collection monitoring answers a different question: are you capturing the proof that auditors will need?
This is the pillar that most organizations neglect, and it is the pillar that causes the most audit-season pain. Your controls may be operating perfectly, but if the evidence of their operation is not being collected and stored, you cannot prove it to an auditor.
Evidence collection monitoring involves watching the evidence pipeline itself:
- Are automated evidence collections completing successfully? A common failure mode is an API integration that stops working after a vendor updates their API, silently producing empty exports for months.
- Are manual evidence items being completed on schedule? If a quarterly access review is due by January 15 and it is January 20 with no completed review, that is a monitoring alert.
- Are evidence items properly formatted and complete? An access review record that lists users but does not document the reviewer's approval and the review date is incomplete evidence.
- Are there gaps in the evidence timeline? If you have monthly vulnerability scan reports for January through September and then nothing for October and November, the auditor will notice.
The meta-level nature of this pillar is important: you are monitoring the monitoring. Your compliance monitoring program must include checks that verify its own evidence collection processes are functioning.
Pillar 3: Risk monitoring
Risk monitoring tracks changes to your organization's risk landscape and ensures that your risk register, risk assessments, and risk treatment plans remain current.
This pillar includes:

- Risk register currency. Monitoring whether identified risks are being treated according to their treatment plans, whether risk acceptance decisions are still valid, and whether new risks have been identified that are not yet in the register.
- Risk assessment triggers. Detecting environmental or operational changes that should trigger a risk reassessment -- such as entering a new market, adopting a new technology, experiencing a security incident, or undergoing a significant organizational change.
- Compliance landscape changes. Tracking changes to regulatory requirements, framework updates, and enforcement actions that may affect your compliance obligations.
ISO 27001 Clause 6.1 requires organizations to address risks and opportunities through a documented risk assessment process. Compliance monitoring ensures that this process is not a one-time exercise but a living function that evolves with the business.
Pillar 4: Vendor monitoring
Your security posture extends to every third party that processes, stores, or has access to your data. Vendor monitoring ensures that your supply chain risk does not silently erode your compliance posture.
This pillar includes:
- Certification currency. Tracking the expiration dates of vendor SOC 2 reports, ISO 27001 certificates, HIPAA compliance attestations, and other certifications. A vendor whose SOC 2 report expired six months ago is a vendor you can no longer rely on for compliance evidence.
- Vendor security posture. Monitoring for publicly disclosed breaches, vulnerabilities, or security incidents affecting your vendors. If a critical vendor announces a breach, your incident response process should activate immediately.
- Contractual compliance. Verifying that vendor contracts, Business Associate Agreements, Data Processing Agreements, and SLAs remain current and enforceable.
- Subprocessor changes. Tracking whether vendors have added, removed, or changed subprocessors that may affect the security or compliance of the services they provide to you.

Pillar 5: Policy compliance monitoring
The final pillar monitors whether your organization is operating in accordance with its own documented policies. Policies are the foundation of every compliance framework -- they define what your organization commits to doing. Policy compliance monitoring verifies that those commitments are being kept.
This pillar includes:
- Policy acknowledgment tracking. Every employee should acknowledge key policies (information security policy, acceptable use policy, data classification policy) upon hire and annually. Monitoring tracks who has and has not completed their acknowledgments.
- Training completion tracking. Security awareness training is required by SOC 2, ISO 27001, HIPAA, and PCI DSS. Monitoring tracks completion rates, identifies overdue personnel, and escalates non-compliance.
- Policy currency. Policies should be reviewed and updated at least annually or when significant changes occur. Monitoring tracks the last review date of each policy and alerts when a review is due.
- Policy exception tracking. When a policy exception is granted (temporary or permanent), monitoring tracks the exception's expiration date, renewal requirements, and compensating controls.

Not everything can be automated, and not everything needs to be. The decision of what to automate first should be driven by a prioritization matrix that considers three factors: frequency, audit impact, and automation feasibility.

What to automate first: the practical sequence
