This scenario plays out constantly in first-time compliance audits. Security controls are in place. Engineers have done good work. But the documentation that proves controls exist, that demonstrates management has approved them, and that shows employees have been trained on them — it's absent, incomplete, or three years out of date.

Policies matter to auditors for a specific reason: auditors validate controls by checking whether there is a documented management intent (the policy), whether that intent is implemented (the control), and whether the implementation is monitored (the evidence). Without policy documentation, even the best technical controls cannot be validated. The control exists; the auditor cannot confirm you intended it to exist as a security measure.

General Policy Requirements (Applicable to All 15)

Before the content of each policy, all policies must satisfy these structural requirements:

  • Version number and date: Every policy must have a current version and last-reviewed date (must be within 12 months for most frameworks)
  • Owner: A named individual or role accountable for the policy's accuracy and enforcement
  • Scope: Who and what the policy applies to (employees, contractors, specific systems)
  • Approval signature: A named executive (CISO, CTO, CEO) with date of approval
  • Review cycle: Statement of how frequently the policy is reviewed (annually minimum)
  • Exceptions process: How to request and track exceptions to policy requirements
  • Acknowledgment tracking: Evidence that employees have read and agreed to the policy

1. Information Security Policy

What it covers: The master governance document. Defines your organization's overall commitment to information security, the scope of the security program, management's role, and the framework under which all other policies operate.

What auditors check:

  • Management's explicit commitment to information security
  • Assignment of security roles and responsibilities
  • Reference to all subordinate security policies
  • Annual review evidence

Common gaps: Written as a one-page statement of intent with no actual content. Does not reference the specific frameworks the organization is pursuing. Has not been reviewed in over 12 months.

2. Acceptable Use Policy (AUP)

What it covers: Governs how employees, contractors, and third parties may use company systems, devices, networks, and data. The foundational document for human-layer controls.

What auditors check:

  • Coverage of company-owned and personal devices used for work
  • Social media and communication guidelines
  • Explicit prohibition of specific behaviors (installing unauthorized software, sharing credentials, accessing systems outside job function)
  • Acknowledgment records proving every employee has read and signed the AUP

Common gaps: Generic AUP that doesn't address remote work, BYOD, cloud storage (personal Dropbox, Google Drive), or AI tools. No acknowledgment tracking.

What auditors check:

  • Documented process for access provisioning and deprovisioning
  • Least privilege principle stated
  • Privileged access controls (admin accounts treated separately)
  • Access review frequency (quarterly for privileged, semi-annually for standard is common)
  • Immediate revocation process for terminated employees

Common gaps: No documented process for revoking access when employees are offboarded. No stated review frequency. Privileged accounts not treated differently from standard accounts.

  • Complexity requirements
  • Prohibition of password reuse (at least four previous passwords)
  • Password rotation requirements for privileged accounts
  • Password manager use (required or recommended)
  • Prohibition of password sharing

Common gaps: Password length requirement below 12 characters. No password manager mandate. No separate rotation requirement for privileged accounts. No process for changing shared/default passwords.

What it covers: How the organization detects, classifies, responds to, and recovers from security incidents and breaches. Required by every major compliance framework.

What auditors check:

  • Incident classification definitions (severity levels)
  • Named incident response team and roles
  • Escalation procedures by incident type and severity
  • Communication procedures (internal, customer, regulatory)
  • Documentation requirements during incident response
  • Post-incident review (lessons learned) requirement
  • Evidence that the plan has been tested (tabletop exercise) within the past 12 months

Common gaps: Incident response plan exists as a document but has never been tested. No regulatory notification timelines specified (GDPR: 72 hours; HIPAA: 60 days; PCI DSS: immediate notification to card brands). No customer communication template. No post-incident review requirement.

What it covers: How the organization categorizes data by sensitivity and the handling requirements for each category.

What auditors check:

  • Defined classification levels (typically Public, Internal, Confidential, Restricted)
  • Specific handling requirements for each level (storage, transmission, disposal)
  • Assignment of data owners
  • Coverage of all major data types the organization handles

Common gaps: Classification levels defined but no handling requirements specified for each level. No guidance on how to handle data received from customers. PHI or PCI data not explicitly identified as a distinct category with specific handling requirements.

  • Documented vendor approval process including security review
  • Evidence of vendor security assessments (security questionnaires, SOC 2 reports, certifications)
  • Contract requirements (DPAs, BAAs, security addendums)
  • Periodic vendor re-assessment schedule
  • Vendor access revocation process on contract termination

Common gaps: No documented vendor approval process. Security review happens informally (a Slack message to the CISO) with no documentation. No tracking of vendor SOC 2 report expiration or renewal. No contract terms requiring vendor notification of breaches.

8. Change Management Policy

What it covers: Controls for managing changes to production systems, applications, and infrastructure to prevent unauthorized or unplanned changes from introducing risk.

What auditors check: Formal change request and approval process. Separation of duties (developer cannot approve and deploy their own changes to production). Emergency change process. Post-change testing requirements. Change log maintenance.

Common gaps: Engineers deploy directly to production without review. No emergency change process documented. Change logs not maintained or not retained for the required period (12 months minimum for most frameworks).

What auditors check: Defined RTO (Recovery Time Objective) and RPO (Recovery Point Objective) for critical systems. Tested recovery procedures. Backup verification. Documented and tested DR plan. Annual BCP/DR test evidence.

Common gaps: RTO/RPO not defined for specific systems. Backups exist but restoration has never been tested. BCP plan exists as a document but has not been exercised. No definition of what constitutes a "disaster" requiring plan activation.

Common gaps: Written before COVID when remote work was an exception; not updated to reflect current reality where remote work is standard. No guidance on co-working spaces or public Wi-Fi. No requirement to report lost or stolen devices within a defined timeframe.

Common gaps: Policy does not specify minimum algorithm standards. No key management requirements. Does not address mobile device encryption or removable media encryption. Does not prohibit deprecated algorithms.

What auditors check: Log coverage (which systems log what events), log retention period (3–12 months minimum depending on framework), log review frequency (daily for PCI DSS), log protection (integrity, no modification), and alerting requirements.

Common gaps: Logs collected but not reviewed. Retention period not specified or below framework minimums. No log integrity controls (logs could be tampered with). No SIEM or centralized logging — logs exist on individual servers.

What auditors check: Vulnerability scanning frequency (quarterly external scans for PCI DSS), patch application timelines by severity (critical: 30 days for PCI DSS), penetration testing schedule (annual minimum), and tracking/remediation evidence.

Common gaps: No defined patch timelines by severity. Scans conducted but findings not tracked to remediation. No penetration testing policy. No process for prioritizing vulnerabilities when multiple are discovered simultaneously.

Common gaps: Policy written for an office that no longer exists (company is now fully remote). No hardware disposal procedure. No visitor log process. Outdated references to physical infrastructure that was migrated to cloud.
