What Is an Access Control Policy?
An access control policy is a formal, management-approved document that defines the principles, rules, and procedures governing how access to information systems, applications, data, and facilities is authorized, managed, and revoked throughout the organization. It is one of the core policies in any information security policy framework and typically the second or third document auditors request after the master security policy itself.
Purpose
An access control policy serves three functions:

Without this documented commitment, your access controls exist as ad hoc technical configurations rather than as governed security measures.

Scope

Why Access Controls Are the Most-Tested Audit Area
Access control is not just one of many audit areas. It is, by a measurable margin, the area where auditors invest the most time and generate the most evidence requests. There is a straightforward reason: access control is where the risk is. If your encryption is misconfigured, that is a vulnerability. If your access controls are misconfigured, an unauthorized person has already accessed the data.
Every major compliance framework devotes substantial control requirements to access management:
SOC 2: CC6.1 through CC6.8
SOC 2's Common Criteria 6 series (Logical and Physical Access Controls) contains eight criteria dedicated to access control -- more than any other control domain in the framework:
- CC6.1: The entity implements logical access security software, infrastructure, and architectures over protected information assets.
- CC6.2: Prior to issuing system credentials and granting system access, the entity registers and authorizes new internal and external users.

ISO 27001: A.5.15 through A.5.18 and A.8.2 through A.8.5

HIPAA: Section 164.312(a)(1) -- Access Control
The HIPAA Security Rule Technical Safeguards open with access control. Access Control Models: RBAC, ABAC, and MAC

Role-Based Access Control (RBAC)

- Directly supports the compliance requirement that access be based on "role and business need"
- Makes access reviews straightforward -- review the role assignments, not individual permissions
Weaknesses:
- Can lead to "role explosion" in complex organizations where hundreds of granular roles are created
- Static -- does not account for contextual factors (location, time, device health)
- Difficult to implement for cross-functional roles that span multiple departments
Best for: Most SaaS companies, mid-market organizations, and any company where job functions map cleanly to access requirements. RBAC is the model most auditors expect to see and the easiest to demonstrate compliance with.
Attribute-Based Access Control (ABAC)
ABAC determines access based on attributes -- properties of the user (department, clearance level, location), the resource (classification, owner, sensitivity), the action (read, write, delete), and the environment (time of day, network location, device posture). Policies are expressed as rules that evaluate combinations of these attributes.
How it works: An ABAC policy might state: "A user in the Engineering department, accessing from a managed device, on the corporate network, during business hours, may read production logs classified as Internal." Change any attribute -- access from an unmanaged device, or outside business hours -- and the rule may deny access.
Strengths:
- Highly granular and context-aware
- Reduces role explosion by using attributes instead of predefined roles
- Supports dynamic, conditional access decisions
- Aligns with zero trust architecture principles
Weaknesses:
- Complex to design, implement, and maintain
- Harder to audit -- the auditor must understand the attribute evaluation logic
- Requires mature identity and device management infrastructure
Best for: Large enterprises, organizations with complex access requirements, cloud-native environments with dynamic workloads, and companies implementing zero trust.
Mandatory Access Control (MAC)
MAC assigns access levels based on information classification and user clearance levels. Users cannot override or modify access decisions -- the system enforces them based on labels assigned to both the user and the data. A user with "Confidential" clearance can access Confidential and lower-classification data but cannot access Restricted data.
How it works: Data is labeled with a classification (Public, Internal, Confidential, Restricted). Users are assigned a clearance level. The system enforces that users can only access data at or below their clearance level. No user -- including administrators -- can override the classification labels.
Strengths:
- Highest level of access control enforcement
- Eliminates discretionary access decisions that introduce risk
- Required in certain government and military environments
Weaknesses:
- Rigid and difficult to implement in dynamic business environments
- Requires comprehensive data classification (which most commercial organizations have not completed)
- Impractical for most SaaS companies
Best for: Government agencies, defense contractors, organizations handling classified information. Most commercial SaaS companies will not implement pure MAC, but the classification principles inform data-handling requirements in policies.
Practical recommendation
For the majority of SaaS companies pursuing SOC 2, ISO 27001, HIPAA, or PCI DSS, RBAC is the primary model. Supplement it with ABAC elements where context-aware decisions are needed (conditional access policies in identity providers like Okta or Azure AD that evaluate device posture, location, and risk signals). Document the model in your access control policy so auditors understand the approach and can evaluate it consistently.
The Principle of Least Privilege: Implementation Guide
If there is one access control concept that appears in every compliance framework, every audit checklist, and every enterprise security questionnaire, it is least privilege. The principle of least privilege states that every user, process, and system should be granted the minimum level of access necessary to perform its assigned function -- and nothing more.
Least privilege is not aspirational guidance. It is an explicitly required control:
- SOC 2 CC6.3: Requires access based on roles and responsibilities.
- ISO 27001 A.5.15: Requires access based on business and information security requirements.
- HIPAA 164.312(a)(1): Requires access limited to authorized persons.
- PCI DSS 7.2: Requires access limited to the least privileges necessary.
Why least privilege is the number one access control requirement
Least privilege limits the blast radius of every security incident. If a user's credentials are compromised, least privilege ensures the attacker can only access what that specific user could access -- not the entire environment. If an insider acts maliciously, least privilege constrains the damage to their assigned scope of access. If a service account is misconfigured, least privilege limits what it can reach.
The inverse is also true: when least privilege is not enforced, a single compromised credential can lead to complete environment compromise. Most major breaches in the past five years involved an initial foothold through a low-level account followed by lateral movement enabled by overly broad permissions.
How to implement least privilege
1. Start with a role matrix. Define every role in the organization and the specific systems, applications, and data sets each role requires access to. Map the required access level (read, write, admin) for each resource. This is the foundation of your RBAC model.
2. Default to deny. Configure all systems so that new users have no access by default. Access is granted explicitly through role assignment, not by inheritance or default permissions. This is the opposite of how many systems ship -- most default to overly permissive configurations.
3. Eliminate standing privileges where possible. For sensitive operations (production database access, infrastructure changes, customer data exports), implement just-in-time (JIT) access: users request elevated access for a specific task and timeframe, access is granted temporarily, and it is automatically revoked when the window expires.
4. Review and right-size regularly. Permissions accumulate. An engineer who started in QA, moved to development, and then to DevOps may carry permissions from all three roles. Quarterly access reviews (covered in detail below) are the mechanism for identifying and removing this accumulation.
5. Apply to non-human identities. Least privilege applies to service accounts, API keys, CI/CD pipelines, and automated processes -- not just human users. A CI/CD pipeline that deploys to production should not have read access to the customer database. An analytics service account should not have write permissions to any system.
Authentication Controls: MFA, SSO, and Password Policies
Authentication controls verify that a user is who they claim to be before granting access. Every compliance framework imposes specific authentication requirements, and the trend across all frameworks is toward stronger, multi-factor authentication.
Multi-Factor Authentication (MFA)
MFA requires users to present two or more verification factors from different categories: something you know (password), something you have (hardware token, mobile device), or something you are (biometric). MFA is the single most effective control against credential-based attacks, and it is explicitly required or strongly expected across every major framework:
- SOC 2: Auditors expect MFA on all production systems, administrative consoles, and remote access. It is tested under CC6.1 and CC6.6.
- ISO 27001 A.8.5: Requires secure authentication technologies commensurate with the sensitivity of the system.

Password Policies
Password requirements vary by framework, but the trend is toward longer passphrases and MFA rather than complex, frequently rotated passwords. NIST SP 800-63B, which heavily influences SOC 2 and ISO 27001 auditor expectations, recommends:

Authentication verifies identity. Authorization determines what that identity can do. Your access control policy must define both.
Building a role-based access matrix
A role-based access matrix is the operational artifact that translates your policy's least-privilege requirements into specific, implementable access grants. It maps every role in the organization to the specific systems and permission levels that role requires.

Key principles for the access matrix:
- No role should have access to everything. If a single role has admin access to every system, you do not have RBAC -- you have a superuser problem.
- Separate duties where possible. The person who deploys code should not be the same person who approves the deployment. The person who creates user accounts should not be the same person who assigns permissions.
- Document business justification. For every access grant in the matrix, there should be a clear business reason. "They might need it someday" is not a justification.
- Review the matrix when roles change. When the organization restructures, creates new roles, or adopts new systems, the access matrix must be updated.
Separation of Duties (SoD)
Separation of duties prevents any single individual from having sufficient access to execute a critical process end-to-end without oversight. It is a specific requirement across all four frameworks:
- SOC 2 CC6.3: Requires segregation of incompatible duties.
- ISO 27001 A.5.3: Requires that conflicting duties and conflicting areas of responsibility be segregated.
- PCI DSS 7.2: Requires access limitations that include separation of duties.
Common SoD controls:
- Developers cannot approve and merge their own pull requests to production.
- The person who requests access cannot be the person who approves it.

Provisioning: Onboarding New Users
When a new employee, contractor, or third-party user joins the organization, access must be provisioned through a documented, repeatable process:

Access request. The hiring manager or department lead submits an access request specifying the user's role, required systems, and business justification. This request should flow through a ticketing system (Jira, ServiceNow, or equivalent) to create an audit trail.

When users change roles within the organization, their old access must be removed and new role-appropriate access must be granted. This is the step most organizations fail at. - When an employee changes departments or roles, the old manager must confirm that prior role access is no longer needed.

User access reviews are the periodic verification that every user's current access is still appropriate for their role and responsibilities. They are explicitly required by every major framework and are among the most scrutinized audit evidence.
What frameworks require
- SOC 2 CC6.3: Requires periodic review of access rights.
- ISO 27001 A.5.18: Requires that access rights be reviewed at regular intervals.
- HIPAA: OCR expects periodic review of ePHI access rights.

How to conduct a quarterly access review
Step 1: Generate the review list. Export a list of all active users from each system in scope, including their role assignments and permission levels. Compare this list against the HR system to identify any discrepancies (active accounts for terminated employees, accounts without an owner).
Step 2: Assign reviewers. Each system owner (or designated reviewer) receives the list of users with access to their system. The reviewer should be the person with business context to evaluate whether each user's access is appropriate -- typically the system owner or department manager.
